Fit (Sick) Notes
Sick Notes (or ‘Fit Notes’ as they’re officially called) can be requested through our online system. Once you have done this, you will usually be sent a short questionnaire to gather further information.
You should go back to work as soon as you feel able to and with your employer’s agreement – this may be before your fit note runs out. You do not need to go back to your doctor before going back to work. The doctor cannot give you a fit note stating that you are ‘fit for work’.
Collecting Fit Notes
We ask our patients to receive fit notes electronically – these have a QR code which your employer can scan to verify it.
If you require a paper fit note, when you have been notified it is ready, you can collect from the front desk any day after 2pm.
Self Certification
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. Full government information is available at this link.

Support with Work
WorkWell is a free, UK government funded initiative designed to support individuals with health conditions or disabilities, whether a mental health or physical health concern.
You can access WorkWell if you’re employed, self-employed or recently unemployed, and are facing health-related challenges to either remain in work or to return to work.
They offer:
- a personal assessment to understand the best support for your needs
- tailored plans to address your physical and mental wellbeing
- access to local health services, wellbeing activities, and community support
- careers advice and guidance to help you progress in your current job or explore new opportunities
- tailored support from a dedicated Work and Health Coach
